What problems would I cause if I stop my Exchange 2000 server from receiving emails with .pif attachments -- I have NAI anti-virus software which updates DATs every day.

IMHO, this will not cause any problems. In fact, I also block all PIF files (and several other file extensions) because at my company we don't have any legitimate business needs to email these file types around. As you know, many viruses (including ones that are making the rounds right now) are packed into malicious PIF files and rely on email transmission and end-user execution to spread the infection. If you block these things at your network border, or anywhere in between the malicious sender and your end-user mailboxes, then you are doing a good thing and protecting your users and network.
